Quality of Life and Care Deficiencies – Parkview Rehabilitation Center was cited in 2022 after a resident with cognitive impairments exited through the double fire doors and stopped mere meters from a busy local road. Nurses failed to respond to an alarm informing them that a resident had left the building, only responding 30 minutes later. During that time, the FL resident could have been hit by a car or fallen from his wheelchair, especially as he had a history of falls. The Winter Park facility maintenance director admitted that the exit alarms were functioning, so there was no excuse for the staff neglecting this resident and allowing him to escape.

Death of a Patient – In 2021, a resident at this Winter Park nursing home fell off the bed and subsequently died from brain damage. A Certified Nursing Assistant (CNA) was performing incontinence care independently; she requested help from another nursing assistant, who told her to wait. She also failed to review the FL resident’s care plan to see whether a two-person assist was needed. The CNA stated that the need to complete care before shift change was why she failed to wait for another staff member to help.

Care Planning Deficiencies – In the most recent inspection from 2023, it was noted that the nursing staff did not provide the proper food for patients with dysphagia, who required pre-thickened liquids to prevent choking. Instead, they were provided with regular water and ice, which posed a severe choking hazard.
