Canyon Transitional Rehabilitation Center, LLC – [9/1/2024] 

  • Violation: Various violations, including resident’s rights and quality of life deficiencies at Canyon Transition Rehabilitation Center.
  • Example: Failure to honor the resident’s right to a safe, clean, and comfortable environment; failure to safeguard and protect confidential resident information; failure to inform residents of their health status and care; failure to honor resident rights.

Bloomfield Nursing and Rehabilitation Center [9/1/2024] 

  • Violation: Various deficiencies related to resident’s rights, quality of life and care, nursing and physician services, and pharmacy service.
  • Example: Failure to safeguard resident information, observe, monitor, and train nursing staff, honor a resident’s right to request, refuse, or discontinue treatment, and ensure each resident receives nutritional diets based on their unique needs and allergies.

River City Center [9/1/2024] 

  • Violation: Numerous violations involving residents’ rights, nutrition and diet, and quality of life deficiencies.
  • Example: Failure to honor the resident’s right to a safe and clean environment. Failing to ensure meals and snacks are in line with the resident’s needs. Failure to dispose of garbage properly.

Casa De Oro Center, Las Cruces, New Mexico [8/6/2024] 

  • Violation: Numerous citations, including freedom from abuse, neglect, or exploitation.
  • Example: Failure to report suspected abuse, neglect, or theft promptly; failure to provide residents with sufficient time before discharge; failure to notify the resident or their representative of facility stay or transfer.

Additionally, many Genesis Healthcare locations have been fined or ordered to shut down for negligent or abusive behaviors toward patients. 

The Genesis Healthcare Quinnipiac Valley Center was shut down after two patients lost their lives. The Department of Health investigated the claim and found that employees weren’t administering medications on time and that the center failed to hire or train employees properly.

Genesis Healthcare Heritage Hall West and Heritage Hall North were fined almost $90,000 by the federal government for inadequate care of residents due to neglect. There were concerns of reckless or negligent care following surgery and other necessary patient treatments.
