Saber Healthcare Group offers rehabilitation, long-term care, skilled nursing, and memory care in Delaware, Indiana, North Carolina, Ohio, Pennsylvania, and Virginia. Although the organization claims that its affiliated communities provide personal attention and proven results, it has been plagued by low ratings, health citations, and lawsuits.

Saber Healthcare Group operates 142 nursing homes in six states and employs over 15,000 staff. It claims to be committed to providing outstanding service to all seniors in a community setting where they can enjoy a full life with their families and friends.

Ownership and Affiliations

Saber Healthcare Group is a for-profit company owned by Bill Weisburg, who serves on the board of the American Healthcare Association and several regional healthcare associations. Weisburg’s co-founder, George Repchick, stepped down from Saber Healthcare Group in 2019 and joined Embassy Healthcare as CEO. 

In 2018, Weisburg filed a lawsuit against the Internal Revenue Service due to tax issues with a loan taken out for Saber Healthcare Group, but the suit was unsuccessful.

The organization is affiliated with the Pennsylvania Healthcare Association, and its PA team gathered at the 2024 PHCA convention for a team dinner. 

Ratings and Performance

The Centers for Medicare and Medicaid Services provides ratings for all nursing homes and rehabilitation centers that take government insurance. There is a five-star rating based on three subfactors: health inspections, staffing, and quality measures. Each overall rating is considered on this scale:

  • 5 stars – Much above average
  • 4 stars – Above average
  • 3 stars – Average
  • 2 stars – Below average
  • 1 star – Much below average

The Centers for Medicare and Medicaid Services may also place a Consumer Alert on certain nursing homes, demonstrating that it has been investigated for abuse or neglect. 

In general, an assisted living community will not accept Medicare; therefore, it will not have a CMS rating. Instead, consumers can use the US News & World Report overview to understand potential issues with the assisted living community.

Complaints and Actions Against Saber Healthcare Group

Sixteen Saber Healthcare Group facilities have a consumer alert for abuse, underscoring the importance of carefully researching a nursing home before placing a family member there. The company has faced significant backlash from regulatory agencies as well as residents. 

Complaints and Citations

According to the Good Jobs First Violation Tracker, Saber Healthcare Group has been forced to pay over $16 million in fines since 2000. 

Its facilities were cited 215 times for healthcare violations. The largest of these penalties was against Autumn Care of Myrtle Grove in North Carolina, fined $284,679 and given a payment suspension for several offenses, including denying residents agency and self-determination. [1]

Lawsuits

United States ex rel. Wright et al. v. Saber Healthcare Holdings, LLC et al

In 2020, Saber Healthcare Group was accused of falsely submitting Medicare claims for rehab services that were not reasonable or necessary. The company settled the case with the Department of Justice for $10 million.

Goldstein v. Saber Healthcare Group, L.L.C.

A patient at the Highland Point Health and Rehab Center in Highland Heights, OH, died after developing sepsis due to negligence. The case went to arbitration, but the decision was appealed, and the case is still ongoing. 

Gay v. Saber Healthcare Group., L.L.C

A patient living at Autumn Care of Raeford in North Carolina died after a series of falls, and her daughter filed a wrongful death suit. Saber Healthcare Group attempted to dismiss the case by claiming the daughter agreed to an arbitration order when placing her mother in care, but the court overturned this.

Inspections and Deficiency Reports at Saber Healthcare Group

Despite significant public relations efforts by Saber Healthcare Group, such as running a food plating contest where affiliated dietary teams learned how to present different foods with regional food service director Chef Byron Brown, details from recent inspection reports do not demonstrate the same care and attention provided to residents. Recent Inspections

January 4, 2024 – Neglect

In a January 2024 report, it was found that the staff of Wadsworth Pointe in Wadsworth, OH, failed to clean urinary catheters or maintain patients’ dignity by covering them when they were out in common areas. [3]

February 23, 2023 – Neglect

In one February 2023 report, Autumn Care of Madison in Virginia, staff were ordered to weigh a resident every day, as the individual had multiple comorbid conditions that made it difficult to eat or drink. They missed numerous days in one week, which could have put the resident at serious risk of malnutrition. [4] 

July 21, 2022 – Failure to Honor Resident’s Rights

Patients can stay in rooms with preferred people, such as friends or spouses. In a July 2022 report, Monroe Health & Rehab Center in Charlottesville, VA, moved a resident to a new room without informing the individual or their representative. The resident had dementia, making it difficult for her to recognize new people. [5]
