Despite being listed by Great Places to Work as the best healthcare company to work for and claiming that it provides the best clinical care available for memory care services, there are significant concerns about the safety of residents in Trilogy Health Services facilities.

Trilogy Health Services, LLC, is a nonprofit healthcare company offering skilled nursing, independent living assistance, and senior living communities in Indiana, Kentucky, Michigan, Ohio, and Wisconsin. 

The company has approximately 10,000 employees taking care of residents across the Midwest.

Ownership and Affiliations

Trilogy Health Services was recently acquired by American Healthcare REIT, an investment firm that purchases skilled nursing, assisted living, and memory care facilities nationwide. 

The deal, which was completed for $258 million, is not without controversy, as a law firm is currently investigating the company for securities fraud claims.

Trilogy Health Services is affiliated with programs considering a resident’s complete well-being. These include the Live a Dream program funded through the Trilogy Foundation, which gives access to new experiences like ziplining, and the Trilogy Travel Club, where their team handles travel arrangements to top destinations. 

Ratings and Performance

Facilities that accept Medicare or Medicaid receive ratings from the Centers for Medicare and Medicaid Services. The rating is out of 5 stars and comprises three sub-ratings: health inspections, staffing, and quality measures. 

When assessing star ratings, the CMS considers them by this scale:

  • 5 Stars – Much above average
  • 4 Stars – Above average
  • 3 Stars – Average
  • 2 Stars – Below average
  • 1 Star – Much below average

A facility accredited in the last 6 months or one that does not have data for the last 12-16 months will have a “No Rating Available.” 

Assisted living facilities, as they do not accept Medicare, are generally not rated by the CMS.

According to the Good Jobs First Violation Tracker, Trilogy Health Services has had 6 healthcare-related violations since 2020. Three were against the White Oak Health Campus in Monticello, IN. The Homewood Health Campus in Lebanon, IN, has had three violations since 2016. [1]

Low Staffing Ratios

In 2023, WCPO-Cincinnati reported on the low staffing ratios at the Gateway Springs Health Campus in Cincinnati, Ohio, where patients complained that staff would fail to provide necessary services, sometimes for hours. [2]

The issue is so severe that the Ohio governor put together a task force to investigate whether companies like Trilogy Health Services are fulfilling their promises as healthcare providers. 

Lawsuits

Holsopple v. Village Green-Trilogy Health Services, LLC

In this case, filed in Ohio in 2023, the plaintiff accused Village Green Health Campus, managed by Trilogy Health Services, of elder abuse, claiming that they misrepresented a contract they provided to her mother, a resident. This case was ruled in favor of Trilogy Health Services. [3]

Techau v. Trilogy Health Services, LLC

This trial began in 2014 after the wrongful death of the plaintiff’s father during his stay at The Willows of Hamburg, where the plaintiff argued that staff recklessly endangered lives through neglect. They were awarded damages, but the opinion was later reversed in favor of Trilogy Health Services. [4]

Office of Inspector General v. Trilogy Health Services

In 2016, Trilogy Health Services agreed to pay $80,000 for violations of the Civil Monetary Penalties Law related to Medicare inducements. The company failed to check that all residents were eligible for Medicare Part D before waiving copays. 

Inspections and Deficiency Reports at Trilogy Health Services

In 2022, USA Today released a report noting that residents at Trilogy Health Services died of COVID-19 at twice the national rate of other facilities, suggesting a culture of neglect. [5]

June 25, 2024 – Resident-on-Resident Abuse

Trilogy Health Services staff at the Oaks At Northpointe facility in Zanesville, OH, failed to report potential abuse between two residents to the proper authorities, putting both residents and staff at risk of injury.

April 5, 2024 – Neglect

At the Taylor Springs Health Campus in Gahanna, OH, a resident suffered a fall that left her with a closed wrist fracture. She was provided only one dose of pain medication, which she stated did not offer any relief. 

April 4, 2024 – Neglect

An inspection of Wellbrooke Of Westfield in Indiana found that staff failed to monitor dialysis patients or ensure that they were having adequate bowel movements. 

February 14, 2024 – Inadequate Monitoring

A patient at Wellbrooke Of Kokomo, IN, lost over 18 pounds in less than a month, or over 10% of their body weight, due to inadequate weight monitoring. 

February 17, 2023 – Death of Patient

A patient at Waunakee Valley Senior Living in Wisconsin died after Trilogy Health Services staff failed to provide life-saving measures, including CPR.
